Ovulation Induction Instructions

There are two available medications to induce ovulation (prepare and recruit eggs for release) – Femara (Letrozole) and Clomid (Clomiphene). Your provider will choose the medication that best fits your clinical needs.

The side effect profile for each medication is very similar. It is common to experience some hot flashes, abdominal bloating, cramping, mood swings, fatigue, and even some dizziness during their use.

There is an increased risk of multiple gestations (3-10% risk of twins and 0.3-0.5% of triplets). This risk increases as the dose increases. We will be utilizing the lowest possible dose to successfully achieve an ovulation.

There is NO risk of increased miscarriage, ectopic pregnancy or birth defects when compared to spontaneous pregnancies.

There is a small risk (less than 1%) of ovarian hyper-stimulation (or over-stimulation). Severe pain should be reported to your provider immediately.

You will need to start the medication on day # 3 – taking 1 pill daily for 5 days.

Utilize an ovulation prediction kit (OPK) to confirm ovulation with a n LH surge and assist in timing of intercourse.

Regular intercourse is encouraged throughout the cycle, preferably daily or every other day as ovulation approaches. Once an LH surge is detected, prediction an ovulation within 36-40 hours, continue to have intercourse to cover this window of fertility.

Communicate your OPK results to the office to assist in planning on the fertility pathway. We may need to adjust the dose of your medication or proceed in a different manner.

We recommend a day #21 progesterone level to confirm ovulation. Call the office once your cycle starts to schedule.

We recommend that you take a home pregnancy test on day #28. Please notify the office of your results.

If the test is negative, we may need to make changes to our plan for the next cycle.

If the test is positive...
If the test is positive, we will want to get you scheduled to begin your obstetric care. Your first intake obstetric appointment is usually 2 weeks after your positive pregnancy test.
